What we Need

What We Need Before Processing your Claim

Most of the information we need can be found on your insurance card or in your benefits booklet. If you are covered under more than one plan, for example through both your own employer and your spouse's employer, please bring the details for both plans so we can coordinate your benefits and maximise your coverage.

  • Policy holder name

    The name of the person who holds the insurance plan. This may be you, a spouse, or a parent if you are covered under a family plan.

  • Insurance ID number

    Your unique member identification number as shown on your insurance card or benefits documentation.

  • Date of birth

    Your date of birth and the date of birth of any dependants being treated, as insurers use this to verify identity and confirm eligibility.

  • Policy or group number

    The number that identifies the specific plan or group benefit package you are covered under, typically provided by your employer or insurer.

    We'll always let you know what's covered before any treatment begins. No surprises, no awkward conversations at the front desk. If there's a gap, we'll walk you through your options clearly so you can make an informed decision.

  • Can I still be seen if I don't have insurance?

    Absolutely. Insurance or not, everyone gets the same care here. We'll give you a clear breakdown of costs upfront so you always know what to expect before anything happens.

  • Do I need to contact my insurance provider before my appointment?

    No. We handle that side of things. Just bring your insurance information and we'll take it from there. The only time you'd need to contact your provider directly is if your plan requires pre-authorization for specific treatments — and if that's the case, we'll let you know well in advance.

  • What if I have coverage under two insurance plans?

    That's actually a good position to be in. We can coordinate between both providers — submitting to your primary plan first, then billing the remainder to your secondary plan. It's called dual coverage and we deal with it regularly, so just let us know at booking.
